General

Q: What if I do not have a computer?

A: We understand and acknowledge that everyone may not own or have access to a computer. If you fall in this category, please do not be concerned. We suggest that you send us an e-mail at info@mylifeguardian.com or call us at 888-698-1716. Every customer's needs will be identified and addressed on an individual basis.

Q: Do I need a unique e-mail?

A: Yes, your e-mail address is used as your login name. Each account must be created with a unique e-mail address.

Q: Why should I carry the ID Badge?

A: You should carry your ID Badge at all times because it contains your contact information, primary emergency contact, vital information and all pertinent information necessary for you, medical personnel and your loved ones to make accurate decisions to your health.

Security

Q: How safe is the information entered on the website?

A: The website is located on secure servers and requires your unique user name (e-mail address) and password allowing you to enter/view your data. Life Guardian makes every effort to assure your data is secure. However, it is your responsibility to maintain the security of your password. Life Guardian cannot be responsible for your data being entered or changed by person(s) who have obtained your password (please see “terms of use”).

Q: How safe is my social security number?

A: Life Guardian. does not use your social security number for any purpose other than an identifier in a search query used by hospitals. Your SSN will be blocked out of our website except for the last four numbers and will only appear on your ID Badge.

Q: What happens if I lose my ID Badge?

A: If you lose your ID Badge, all of your information is still available on the website and accessible with your user name (e-mail address) and password. Your access to reprint your badge is unlimited during the duration of your membership.

Membership

Q: How long is my membership with Life Guardian?

A: Your membership with Life Guardian will last for one year. However, you can renew your subscription annually which will ensure your data will continue to be stored on the website and that you can continue to update your data whenever your information changes. As a member you will also be able to use any new standard features added to the site.

Q: What happens if my subscription expires?

A: Prior to your membership expiration you will receive an e-mail notice to your e-mail address of record with Life Guardian. You will be told your membership expires within a time frame and given a link to the website where you can renew. On the expiration date of your membership you will receive another e-mail notice that your subscription has expired and again give you an option to renew. If you have not renewed your subscription within one month of its expiration all of your data stored on the site will be removed and archived. Access to your data after that point will require reinstitution of your membership and additional charges for retrieval of your information from archives and reinstalling it on the web site. Memberships not renewed within twelve months of their expiration will result in permanent deletion of the data.

Q: Why should I renew my subscription?

A: The information entered on your file on the Life Guardian website can be a permanent repository. Renewal of your membership assures that information entered will be maintained. It also allows you to continue to make changes, additions and maintain information on your ID Badge. Members will also have full access to the website’s features.

Q: How do I cancel my subscription?

A: A full refund can be received if the cancellation occurs within 15 days of purchase. You can request cancellation by contacting us via e-mail at info@mylifeguardian.com or calling 888-698-1716.

Q: How will I be billed?

A: Your initial membership purchase will be billed to your credit card, via PayPal when you have completed your product or service selection, reviewed and submitted your order.

Family Members

Q: How can I add family members?

A: You can add up to four family members based on the product purchased.
There are single, dual, and family of four packages available. Click here to view product and service page.

Q: What information can I add for family members?

A: You can add personal information including photo, emergency medical information and medical history just as you can for your membership.

Q: Can family members view and edit their own information?

A: Yes, family members can access their own information if purchased by someone else. It is up to the purchaser to provide the intended family member the user name (e-mail address) and password. Access can be accomplished from any web enabled PC. They can add to and edit their information. They can also allow medical personnel to view their information. For family members there is no view only access for medical personnel so it is suggested the password for family members be changed after it is shared. It is up to the purchaser to secure their user name and password. Life Guardian is not responsible for lost, stolen or misused user names and/or passwords.

Q: Why are family members granted access to the site?

A: Having access to their information on the site means in a medical emergency or when seeing a physician while traveling, they will have medical information immediately available. Members have found this particularly useful for children away at school or for parents who may reside in another town or state where the member has some responsibility for their medical care.

Q: Why must family members have a valid e-mail address and password?

A: Access to the site is governed by a user name (e-mail address) and password. The user name must be unique to insure access is not granted to ineligible individuals because of duplication of user names. A valid e-mail address assures there will be no duplication.

Web Site

Q: Do I have to enter all of my data before I can print ID Badge/Medical Data Form?

A: Yes. Your ID Badge will not be beneficial if all data has not been entered. Reminder, the Life Guardian website serves as a database housing all your pertinent information in the event your ID Badge is not readily available. However, you will be able to enter information, save, and return to complete at a later date prior to printing.

Q: Can I make multiple additions or changes to my data on my web site?

A: You can make unlimited changes or additions to the information, on the Life Guardian website, for the duration of your membership. REMINDER: Always reprint your ID Badge/Medical Data Form after any changes or additions.

User Name/Password

Q: How do I establish/manage my passwords?

A: Your passwords are created once the decision to purchase a membership has been established. You will be requested to input contact information, an e-mail address and a password. We suggest that you use passwords that are a random combination of letters and numbers. Your user name is always your e-mail address.

Q: What if I forget my password for the web site?

A: If you cannot remember your password, click on the “forgot your password?” link in the Life Guardian Administration section and a replacement password will be sent to your e-mail address on record. If you are still unable to gain access to your file please e-mail info@mylifeguardian.com, giving your full name and you will be contacted and assisted with your logon.

Access

Q: What if I get to the doctor's office or hospital and don't have my ID Badge?

A: All of the information on your ID Badge is stored on the website for the duration of your membership. Thus, if you arrive at the doctor’s office or hospital and do not have your ID Badge, if the office has access to the internet, present your Wallet ID to the medical personnel. The access codes located on your Wallet ID will enable nurses and physicians to have speedy access to all your information.

Q: I was logged onto Life Guardian’s website (www.mylifeguardian.com), and I was kicked out of the system. WHY?

A: After thirty (30) minutes of inactivity on the keyboard your connection to the Life Guardian website is automatically terminated to protect your information. The timer starts from your last keystroke so it should not produce a problem in normal usage situations. If you are automatically kicked out you will have to reenter any information typed after your last save before you were booted.

SUGGESTION: Continually save your information while on the website.

Q: Where can I purchase My Life Guardian products and services?

Life Guardian offers products and other materials to help ensure your medical information is immediately available and recognizable when it is needed.

Terms of use

The following are the terms and conditions for use of Life Guardian’s products and service. This includes present and future features offered by Life Guardian and any other service that may be offered by Life Guardian for use with your Life Guardian account. Please read them carefully:

This service is provided to individuals who are at least 18 years old or to minors who have parental consent to use the service.

The service is provided for individual personal use only.
Any unauthorized commercial use of the service, or the resale of its services or products, is strictly prohibited.

By purchasing a membership with Life Guardian, you agree to be bound by the terms outlined in this Service Agreement. Life Guardian may, at its sole discretion, immediately terminate service without notice, should user's conduct fail to conform to these conditions. Life Guardian’s remedies with regard to breach of these terms are not limited to termination of service.

Life Guardian is an internet-based business and we use the internet to send and receive certain types of communications.

You agree to be bound by internet regulations, policies and procedures.

You agree to comply with all applicable local, state, national and international laws and regulations.

You agree to not use Life Guardian for illegal purposes; to not interfere or disrupt networks connected to this service; and to comply with all the rules, policies and procedures connected to this service.

You will ensure information entered on Life Guardian site is true and accurate to the best of your ability.

You will not create a false identity for the purpose of misleading others as to the person's medical history, treatment wishes or other information.

You will not attempt to alter the Life Guardian website or products.

You will not attempt to reverse engineer any of Life Guardian’s services or products or otherwise emulate the functionality of Life Guardian's services or products.

Account Termination

Life Guardian may immediately terminate your access to the service and any related service(s) for failure to pay for service. Service is sold in renewable one-year memberships. Failure to renew service under terms and conditions disclosed elsewhere on the site will result in termination of service and access to the site at the end of your initial membership. At least one month's notice of such termination for failure to renew will be given to your e-mail address of record with Life Guardian. Life Guardian will delete the contents of your account on Life Guardian's website after termination. Members whose accounts are terminated may retain their products ordered and purchased and any information on them at the time of termination. Use of information remaining on those products is the sole responsibility of the former member.

Liability

Life Guardian does not guarantee Life Guardian's service will be uninterrupted or error-free. Life Guardian does not guarantee that the uses of its service, or the materials provided within the service, are accurate, without error, or reliable. However, Life Guardian will make every reasonable effort, as defined by Life Guardian, to reliably provide a high quality service.

Life Guardian provides its service "as is". The company makes no warranty or covenant in regards to the availability, reliability, or timeliness of the service. Life Guardian is not responsible for the appropriateness of the service for any purpose. Life Guardian disclaims any implied warranty of merchantability or fitness for a particular purpose relating to the services or any goods provided in connection with the services.

Life Guardian will not be held responsible for any damages whatsoever as a result of the loss of use, data or profits connected to the performance of Life Guardian or the products it provides. The user is solely responsible for any and all information they enter on the Life Guardian servers and/or prints on their products.

Life Guardian does not offer legal or medical services or advice. Advance Directives created as part of Life Guardian's Service are licensed from the National Hospice and Palliative Care Organization (NHPCO). NHPCO has made every effort to insure the appropriateness of the documents, but Life Guardian may not be held liable for their suitability or accuracy. The member/user is solely responsible for insuring the appropriateness of information stored or documents produced with third party professionals of their choice.

The sole remedy for claims by users is to discontinue their use of Life Guardian and its related website(s).

Indemnification

As a condition of your use of the service, you certify to Life Guardian you are solely responsible for data entered and stored on the Life Guardian web site or downloaded to your flash drive. Further, you acknowledge Life Guardian is not responsible for the accuracy of data entered or any subsequent use of that data. You and your heirs and assigns will hold harmless Life Guardian, its assigns, employees or agents for any real or consequential damages which may be incurred by you as a result of the use of this service.

You agree that Life Guardian, its parents, subsidiaries, officers, employees and affiliated companies, cannot be held responsible for any third party claim arising out of your use of the service. You agree to hold harmless Life Guardian for any demand or claim for damages, including reasonable attorneys' fees.

Third party sites, advertisers and promotions

Life Guardian provides links to third party sites deemed to have potential interest to our members strictly as a convenience to our members. The links included within the website may allow you leave the Life Guardian site and visit third party sites. These third party sites are not under the control of Life Guardian. Life Guardian is not responsible for the contents of any third party site nor is Life Guardian responsible for the functionality of any third party site. A link's inclusion in the Life Guardian site does not constitute an endorsement by Life Guardian of the sites or their operators. Be advised, members visiting linked sites are subject to the Terms of Use and Privacy policies of those site(s).

Any promotions sponsored by Affinity Partners (e.g., companies licensing services of Life Guardian for use by their customers or prospects) of Life Guardian or dealings with Affinity Partners are solely between you and that Affinity Partner. Your dealings with Affinity Partners, or their products or services, are solely at your discretion and Life Guardian will assume no liability with regard to your transactions with Affinity Partners. Members receiving Life Guardian services as a result of their relationship with an Affinity Partner are members of Life Guardian. As such the member's information is private and personally identifiable information will not be shared with Affinity Partners.

MODIFICATIONS TO TERMS OF SERVICE, MEMBER POLICIES

Life Guardian reserves the right to review and change this agreement regarding the use of the service at any time and to notify you by posting an updated version of the agreement on this Web site. You are responsible for regularly reviewing Life Guardian policies. Continued use of the Service after any such changes shall constitute your consent to such changes.

Life Guardian may make improvements and/or changes to the service at any time without notice. Any improvements/changes to the standard service will immediately be available to all members in good standing. If premium service is established members with premium service will receive immediate access to any improvements/changes to the premium service in addition to any improvements/changes to the standard service.

GOVERNING LAW

This agreement shall be governed by and construed under the laws of the State of Illinois. Venue and jurisdiction shall be in the State of Illinois.

Privacy Policy

At Life Guardian, we are strongly committed to protecting your privacy. Your privacy is important to us. To protect your privacy, we provide this notice explaining our online information practices and the choices you can make about the way your information is collected and used. You are responsible for the privacy of your information stored on our secure web site through your control of your passwords governing who has permission to view your data (medical professionals and others) and your user name (e-mail address) and password which is required to add to or edit your information.

The Way We Use Information

We store the information you provide about yourself in a secure database where it is maintained for your use and the use of those to whom you provide read only access (medical professionals, etc.). The information is stored for as long as you are an active member unless you request that it be removed. If you allow your membership to lapse deletion of your information will be handled in accordance with our Terms of Use.

We use non-identifying and aggregate information about the use of our Web site to improve the navigation, content, and design of our site.

This information may include, for example, the most and least requested pages and the number of users from the U.S. and other countries, among other things.

We may provide third parties with non-identifying, aggregate information about the use of our web site, but we will never divulge your personally identifiable information.

Finally, unless otherwise noted below, we would not use your personally identifiable information provided to us online for purposes other than those you requested.
